EMERGENCY INFO AND IMMUNIZATION FORM (State Required)
Please click on this link to obtain the most current state required Emergency Information and Immunization Form. Please complete this form and bring it to our office by the first day of school. Please note, this form is required in order to continue attendance at NVCE.

MORNING CARE REGISTRATION FORM
Please click on the link to download the Morning Care Form. This form is required prior to attending our Morning Care program which begins at 7:00 am.
SIBLING MORNING CARE REGISTRATION FORM
If you have a child in Grades 3-6 who begins school at 8:40, all younger siblings in grades Preschool through 2nd grade may be dropped off at 8:30 in Room 9, Building A. There is no charge for this program. Please use this form for registration for this program.
AFTERCARE INFORMATION & REGISTRATION FORM
